Evaluation of MR Spectroscopy and Diffusion Imaging in the Diagnosis of Creutzfeldt-Jakob Disease

Project Aims

The principal objectives of the project are:

  • to determine the benefit of using multi-modal Magnetic Resonance Imaging (MRI) analysis of the brain, which associates Magnetic Resonance Spectroscopy (MRS) and Diffusion-Weighted MRI, in the diagnosis of the Creutzfeld-Jakob Disease (CJD),
  • to analyse if such anomalies vary with the type of disease (sporadic, genetic, iatrogen or new-variant).
  • to study along two subsequent examinations the evolution of anomalies in different types of CJD,
  • to establish, each time a post-mortem examination of the brain is possible, the correlation between the anomalies observed in multi-modal MRI and in neuropathological data.

The study aims to determine the role of adding MRS to the conventional imaging modalities towards an earlier CJD oriented diagnosis. Some facts to be considered are the metabolic indications of neural destruction in the areas that appear to be normal in conventional imagerie; and possible relevant signs of classification of the different types of CJD.

Project Results

  • the accurate detection of abnormal MR intensities (prompted by the clinicians) in the deep grey nuclei of the brain of CJD patients;
  • the study of assymetries in CJD MR signal;
  • the classification of intensities in sporadic CJD FLAIR images with a more significant hypersignal in the caudate nuclei (10/10 cases) and putamen (6/10) than thalami;
  • the quantification of the higher hyperintensities in the pulvinar of variant CJD patients, even in questionable cases;
  • the definition of new MR-based measures to differentiate between CJD cases and healthy controls and between sporadic CJD and variant CJD cases;
  • the quantification of metabolite variantions in spectroscopy data between CJD patients and healthy controls.
